COVID Consent Form
On-site antigen COVID testing is available to students and staff. USD 411 uses the BinaxNow testing platform. This test is a simple, painless anterior nasal swab.
Testing can be used to rule out COVID in the event of possible symptoms. It can also be used for screening close contacts, allowing for continued safe, in-person learning following exposure.
Testing will not occur without parental consent.
